    A Family Care team is now open to people in the Deer Lake and White Bay areas

    Patients are registered on a first come, first served basis.

    A family care team in the Deer Lake and White Bay area is now operational and open to residents. Those who do not have a primary care provider – or won’t have one in the next three months – in the area can register with Patient Connect NL online at: patientconnect.nlchi.nl.ca  or via phone at 1-833-913-4679. Patients are registered on a first come, first served basis. Patient Connect NL is a provincial list of individuals without a Primary Care Provider (Family Doctor or Nurse Practitioner) in Newfoundland and Labrador (NL). The information provided on Patient Connect NL will be used to plan and improve access to primary health care in NL, with the goal of connecting individuals to a Family Care Team or a Primary Care Provider as soon as one becomes available in your area.
